So I am slowly chipping away at several ideas tied to this Title.

Setting:
Earth like world in a slightly more modern time than our current world, with the linch-pin that the world is in trouble via extreme weather and environmental failure, so those inhabitants not trying to live on the land (think pioneer age Americas crossed with the Wasteland or Priest and the Cursed Lands of Judge Dredd/Dredd) lve in (for the lack of a better term) "Mega cities".
At this point there are about 3 "classes" of humans . . . The Top Tier and the most privileged, usually holding positions of power with in the authoritarian government system. (Still working on a name that would tie into the Tunnel Rat theme.) The Second Tier which age basic wage slaves (think high middle class to just above the poverty line.) They work jobs, and have some financial means to support themselves, though in some cases just barely.
The Last tier are the "Tunnel Rats" the poor, destitute and displaced who survive by scavenging the maze like tunnel support systems in and under the cities.

With in the Tiers are also a couple tweaks, as genetic mutation to a degree has occurred (various reasons both man-made and environmental) These tweaks I have two kinda worked out which are Demons and Angels.

Demons are normal appearing humanoids who have the ability to "overcharge" their physical bodies through rage/anger. Sometimes this is controlled other times it is random, but the main effect is pain tolerance goes up along with strength endurance and speed. (See Viking Berserker).

Angels are also normal appearing humanoids how ever they have an "ethereal" quality that allows them limited mental manipulation of electrical energy (electrical-telekinesis) flows around them, as well as limited range though projection.(telepathy)

Of course there will be the normal humanoids not sure what to "label" them as that would fit into the other terms.

Also thinking of having "groups' that tie with emotions like "Sorrows" which would be like Angel's but with a much more focused or limited power set. In the case of "Sorrows" they would be able to overwhelm individuals with sadness or depression . . . I'm almost temped to use ideas like "Sorrows" to represent a more other worldly feel, as if people who have passed in a manner tied with that type of emotion (ie depression, fear) become sort of like a ghost/wraith using that emotional energy to not only attack but also feed and sustain themselves.

The game focus will be on players taking the role of Tunnel Rats and their attempts to survive, live and succeed. If I do it right even if sessions move out of the tunnels there should still be a feeling/thought of tightness as the buildings, streets and so on will all be packed onto each other. So even if you do make it out of the Tunnels you still end up being a Rat of sorts.
