help with web scripts

help with web scripts

Hi guys

I've started creating a character for a virtual tabletop version of DnD. Part of the GMs process is to ask you to complete a questionnaire. however it's a pain its in plain text, requiring you to enter your answers in a different colour or something.

Does anyone have any experience of creating a html/php/javascript version of questionnaires? the problem is that it's not multi choice it's short response.

i.e.
Describe your characters sleeping quarters.

Any help appreciated.

If you already use Google Docs, you could use their forms functionality:

I didn't knew you could do that with Google Docs!

For my forms I use , in there are lot of customizable templates, including like the one you're asking. The problem here is that every single DM will need to create a new form or am I understading it wrong?
